As for mine RP - it can be done with an adult, but the main heroine might be 17-18, since I know you prefer that age; the main idea is that she's regressed and treated as a baby in an emberrassing way.

So that's the idea - the elder teen/young adult works as a babysitter (not necessery to adults, though it's possible; up to discussion) and is strict and even a bit cruel to her wards, treating them as baby-sih as possible. But she fails her exams or does something else really naughty and now is subjected to the treatments she usually delivered.
I know diapers and recatl thermometers are out of question, don't take too much from the pic, still it's meant to be rather humiliating and intense, so take it in account (btw, I'm ok with anyposition for that plot).
