Of course he wasn't trying to hold back on his opinions. Spock was a proud Vulcan, and believed that his intellect surpassed that of any human, his opinions were always valid, and more to the point - correct.

The Vulcan answered, shaking his head a little, his tone taking on the barest trace of attitude, it was noticeable for a Vulcan anyway. At the glare Bones gave him, Spock rose both eyebrows and took a step back. It was that point that Bones snapped at him to get the anti toxins and the Vulcan retrieved it swiftly, his heart rate quickening the slightest bit. He watched as Bones operated and then the patient seemed to be fine

A ghost of a smirk played on the Vulcan's thin lips and he raised his chin a little, oh he looked might cocky at that point, considering if he hadn't been there to offer his assistance, the patient could have died.
