What do you want to see in a spanking game?

I personally don't like children to be the main character cause there is just a very limited amount of things you can do and I like my variety. I think that it would probably be best to have our hero in their 18's or above since he did mention there would be NSFW stuff in this game.

For the NSFW content that could be interesting, maybe after you lose a fight to a certain boss, the enemy could spank you and then toy with your genitals cause she is a succubus or a kitsune or some other sexual horny race. Since this is a CYOA, if you are going to have a boss like that then you should probably give the player some way of working out their problem with her in both an aggressive and a passive way.

I haven't seen a game where the player can be imprisoned and receive dungeon punishments.Maybe the player could get thrown in the dungeon and after getting caned/strapped they could go on quest to reduce their prison sentence.

Public spanking isn't very common either. One thing I've rarely seen spanking fiction deal with is how the spanking effects their reputation. Some witnesses could laugh at the player and tease them and other could feel bad for the player and try to console them afterward.

Coincidentally enough I was just thinking about @redcheek's idea. Godspeak's game seemed like it was going to go down that direction with one of the spanking scenes, but that hasn't been implemented yet.

But there are a crap tonne of Adult games on the net, very few involve spanking... That isn't just clicking a random celebrities butt over and over... or swat a strange monkey doll :S

Playful, consentent unless it's a defeat scene (if you loose a spanking are triggered) but in any case it should be avoidable if you( or the NPC) don't go into the danger

Maybe one way to implement playing as a child would be to have the player be able to use magic to possess or shape-shift into them.

Maybe in-universe there could be spells that change your gender or age temporarily.Like transforming into a female to seduce someone or turning into a kid learn more about rumors around an elementary school.

Either scenario could lead to you getting punished in the possessed/polymorphed persons place or escaping in time and seeing them get spanked for your actions.

Taking the form of a kid could get you spanked by their parents or teacher whereas possessing a maid and stealing plot relevant documents then depossessing her in time could get her in trouble so the next time you see her she's getting caned by her boss.

In line with much of the above, I prefer to see disciplinary spankings and such in spanking games... stories and art, too, for that matter, but aren't games just a combination of the two?
